What is trigger thumb?


Trigger thumb is a condition where a child’s thumb gets stuck in a bent position and may “click” or “lock” when straightened.


·       It is caused by a tight band (called the A1 pulley) at the base of the thumb that prevents the tendon from moving smoothly.

·       Sometimes a small bump (nodule) can be felt at the base of the thumb.

·       It usually affects one thumb, but sometimes both can be involved.


How common is it?


·       Trigger thumb is common in children.

·       It can appear in toddlers or preschool children.

·       It is not often caused by injury.


What problems can it cause?


·       The thumb may stay bent and be difficult to straighten.

·       The child may not be able to fully open the hand.

·       Occasionally the thumb is painful to straighten, but often it is just stiff.


Does it get better on its own?


In some children, trigger thumb improves without treatment, especially if it appears before the age of 1.

·       About one-third of cases improve naturally.

·       If the thumb is still stuck after age 3–4, it is less likely to get better by itself.


What are the treatment options?


1. Observation

·       If your child is very young (under age 2) and the thumb is not painful, it is worth waiting to see if it improves naturally.


2. Surgery (Trigger Thumb Release)

·       If the thumb stays bent, is painful, or does not improve with time, surgery may be recommended.

·       The operation releases the tight pulley at the base of the thumb so the tendon can glide freely again.

·       This is a short procedure, usually done as day surgery under a short general anaesthetic (your child is asleep).


Benefits of surgery


·       Straightens the thumb.

·       Restores full movement and function.

·       Prevents long-term stiffness or deformity.


Risks of surgery


All operations have some risks, although most children recover well. These include:

·       Infection.

·       Bleeding or swelling.

·       Scar tenderness.

·       Damage to nerves or blood vessels near the tendon (rare).

·       The problem coming back (rare).


Recovery after surgery


·       Your child will usually go home the same day.

·       A small dressing will cover the thumb for 1–2 weeks.

·       Stitches are often dissolvable, and rarely need to be removed after 1–2 weeks.

·       Your child can usually start using the thumb normally quite quickly.

·       Full recovery is expected within 4-6 weeks.


Looking after your child after surgery


·       Keep the dressing dry and clean for 2 weeks after surgery.

·       Give pain relief (paracetamol or ibuprofen) if needed, normally only for the first 5-7 days.

·       Encourage gentle use of the thumb after the dressing is removed.


When to seek help


Contact my team if you notice:

·       Redness, swelling, or discharge from the wound.

·       Fever or your child seems unwell.

·       The thumb becomes very painful or does not move as expected.

Will my child’s thumb be normal after surgery? Yes, most children regain full use of the thumb without long-term problems.


Will it affect their growth? No, the surgery only releases the pulley and does not affect bone growth.
